maximums. A medical questionnaire is required to qualify. |

Precision® provides basic coverage and is a practical alternative
for the services and treatments not covered by the government. This coverage is
guaranteed and no medical questionnaire is required. |
| Prism
Continuum® focuses on providing coverage for those leaving a company
group health plan. Coverage is guaranteed, with no medical questionnaire required
if you apply within 60 days of losing your company group benefits. |
